Leapfrog Announces Spring 2022 Leapfrog Hospital Safety Grades

Cleveland, OH May 10, 2022 – The Leapfrog Group, a national nonprofit representing hundreds of the nation’s most influential employers and purchasers of healthcare, announced today the spring 2022 Leapfrog Hospital Safety Grades. The Safety Grade assigns a letter grade to nearly 3,000 U.S. general hospitals based on over 30 measures of patient safety.

The independent grading system assigns an “A,” “B,” “C,” “D,” or “F” grade to all general hospitals across the U.S. based on over 30 national performance measures reflecting errors, accidents, injuries, and infections, as well as systems hospitals have in place to prevent harm. Health Action Council is the Regional Leader for The Leapfrog Group in Minnesota, Ohio, and Oregon.

In Minnesota, 44 hospitals were graded, and 11 earned an “A,” 5 earned a “B,” 23 earned a “C,” 5 earned a “D,” and 0 earned an “F.” 

In Ohio, 116 hospitals were graded, and 44 earned an “A,” 31 earned a “B,” 39 earned a “C,” 2 earned a “D,” and 0 earned an “F.” 

In Oregon, 34 hospitals were graded, and 16 earned an “A,” 9 earned a “B,” 8 earned a “C,” 1 earned a “D,” and 0 earned an “F.” 

The hospitals that earned an “A” in Minnesota include Abbott Northwestern Hospital, Buffalo Hospital, Hutchinson Health Hospital, Mayo Clinic Health System - Austin, Mayo Clinic Health System - Fairmont, Mayo Clinic Health System - Mankato, Mayo Clinic Health System - Red Wing, Mayo Clinic Hospital - Methodist Campus, Mayo Clinic Hospital - Saint Marys Campus, Methodist Hospital, and Olmsted Medical Center. 

The hospitals that earned an “A” in Ohio include Adena Regional Medical Center, Berger Hospital, Bethesda Butler Hospital, Bethesda North Hospital, CHWC Bryan Hospital, Cleveland Clinic Avon Hospital, Cleveland Clinic Health System - Fairview Hospital, Cleveland Clinic Health System - Hillcrest Hospital, Cleveland Clinic Health System - Lutheran Hospital, Cleveland Clinic Health System - Medina General Hospital, Cleveland Clinic Health System - South Pointe Hospital, Cleveland Clinic Mercy Hospital, Clinton Memorial Hospital, Coshocton Regional Medical Center, East Liverpool City Hospital, Firelands Regional Medical Center, Good Samaritan Hospital of Cincinnati, Jewish Hospital - Mercy Health, Kettering Health Hamilton, Kettering Health Miamisburg, Kettering Health Soin Medical Center, Kettering Health Troy, Licking Memorial Hospital, Mary Rutan Hospital, McCullough-Hyde Memorial Hospital, Memorial Hospital, Mercy Health - Anderson Hospital, Mercy Health - St. Anne Hospital, Mercy Hospital - Defiance Hospital, Mount Carmel St. Ann's Hospital, OhioHealth -Marion General Hospital, OhioHealth - O'Bleness Hospital, OhioHealth Doctors Hospital, OhioHealth Dublin Methodist Hospital, OhioHealth Grady Memorial Hospital, OhioHealth Mansfield Hospital, OhioHealth Riverside Methodist Hospital, ProMedica Flower Hospital, ProMedica Toledo Hospital, Southern Ohio Medical Center, St. Rita's Medical Center, The Christ Hospital, University Hospitals Samaritan Medical Center, and Upper Valley Medical Center.

The hospitals that earned an “A” in Oregon include Asante Ashland Community Hospital, Asante Rogue Regional Medical Center, Asante Three Rivers Medical Center, Kaiser Permanente Sunnyside Medical Center, Kaiser Permanente Westside Medical Center, Legacy Meridian Park Medical Center, Legacy Mount Hood Medical Center, PeaceHealth Sacred Heart Medical Center at RiverBend, PeaceHealth Sacred Heart Medical Center University District, Providence Medford Medical Center, Providence Milwaukie Hospital, Providence Portland Medical Center, Providence St. Vincent Medical Center, Santiam Hospital, St. Charles Health System - Bend, and Willamette Valley Medical Ctr.

The Leapfrog Hospital Safety Grade is the only hospital ratings program based exclusively on hospital prevention of medical errors and harm to patients. The grading system is peer-reviewed, fully transparent, and free to the public. Grades are updated biannually in the fall and in the spring.
