Fall 2025 Leapfrog Hospital Safety Grades Released - Hospitals Earn an "A" Grade

The Leapfrog Group today released its fall 2025 Hospital Safety Grade, an “A,” “B,” “C,” “D” or “F” assigned to all general hospitals in the United States based on their ability to protect patients from medical errors, accidents, injuries and infections. The Grades give the community critical insight about local hospitals. Hospital Safety Grades are updated twice per year by The Leapfrog Group, an independent national watchdog nonprofit.

Health Action Council is a Leapfrog regional leader for Minnesota, Ohio, and Oregon. In Minnesota, 8 hospitals earned an A, 8 hospitals earned a B, 22 hospitals earned a C, and 7 hospitals earned a D grade. In Ohio, 26 hospitals earned an A, 31 hospitals earned a B, 47 hospitals earned a C, and 8 hospitals earned a D grade. In Oregon, 9 hospitals earned an A, 6 hospitals earned a B, 10 hospitals earned a C, and 7 hospitals earned a D grade.

“Preventable medical errors remain a significant challenge in hospitals across the nation, and far too many people are harmed or killed every year as a result. A Leapfrog analysis found that 90% of hospitals with a fall 2025 Safety Grade are part of a health system. Among A hospitals, the chance of being system affiliated is slightly higher, at 94%. (“Health System” is defined as a network of health care facilities that are owned or managed under a single parent organization).

Fall 2025 Safety Grade data shows 358 hospitals nationwide that not only achieved an A for fall 2025 but sustained the grade for more than two years. This select group, representing 13% of all eligible hospitals, is highlighted on www.hospitalsafetygrade.org.
