Health Action Council Ohio (HAC) provides this notice to explain its practices regarding collection and use of personal information through this website (the "Site"). HAC will not disclose, sell, or rent any of your identifiable personal information to any third party, unless approved by you, or required by law. If you are a registered user of HAC Web site, subscriber to HAC publications, or HAC member, you may review and update/correct your information by using your user name and password in the Member Center of this site, or by contacting HAC at (216) 328-2200.

To opt out of an e-mail list, click the "unsubscribe" link at the bottom of any e-mail newsletter (Member Memo) from HAC. You can also call HAC at (216) 328-2200.
With whom we may share information?
HAC may share personal information with our business partners in order to provide them an opportunity to offer products or services that may be of interest to you. HAC also occasionally hires other companies to provide limited services on our behalf including, but not limited to, packaging, mailing and delivering purchases, answering customer questions about products or services, consulting services, data modeling, printing, sending postal mail and processing event registration. We will only provide those companies the information they need to deliver the service. They are prohibited from using that information for any other purpose.

Unless specifically stated on registration materials, the deadline to receive a refund for an individual registration is seven business days before the event. Cancellations received after the stated deadline will not be eligible for a refund. Refunds will not be available for registrants who choose not to attend an event. Cancellations will be accepted in writing only and must be received by the stated cancellation deadline. All refund requests must be made by the attendee or credit card holder. Refund requests must include the name of the attendee and/or transaction number. Refund requests must be submitted via email to [email protected].
