The Leapfrog Group, an independent national nonprofit focused on patient safety, released its Spring 2025 Hospital Safety Grade. The biannual Safety Grade is an “A,” “B,” “C,” “D” or “F” assigned to all general hospitals in the United States based on their ability to protect patients from medical errors, accidents, injuries and infections. The grades give the community critical insight about local hospitals.
Health Action Council is a Leapfrog regional leader for Minnesota, Ohio, and Oregon. In Minnesota, 4 hospitals earned an A, 5 hospitals earned a B, 29 hospitals earned a C, and 6 hospitals earned a D grade. In Ohio, 28 hospitals earned an A, 21 hospitals earned a B, 53 hospitals earned a C, and 11 hospitals earned a D grade. In Oregon, 7 hospitals earned an A, 6 hospitals earned a B, 12 hospitals earned a C, and 8 hospitals earned a D grade.

For the first time, Leapfrog is spotlighting the 346 hospitals nationwide that not only achieved an A for spring 2025 but sustained the grade for more than two years. This select group, representing 12% of all eligible hospitals, is now highlighted on www.hospitalsafetygrade.org.

The Grades are determined by public, peer-reviewed methodology, calculated by top patient safety experts under the guidance of a National Expert Panel.
