Preventable medical errors kill more than 250,000 people every year. Health Action Council recognizes the impact these errors have on employers and their workforce

Hence in 2005 Health Action Council led a Leapfrog rollout in Ohio and expanded our role in 2015 as a Leapfrog coordinator in Oregon and Minnesota.

Through its Hospital Safety Scores, the Leapfrog Group has created and deployed a grading scale that rates hospitals on their overall patient safety efforts and outcomes. The scores provide a level of quality transparency that allows employers and employees to take an active role in controlling treatment outcomes and unnecessary costs by avoiding facilities that have higher rates of hospital acquired infections, errors, accidents, and injuries. Payors, employers, and purchasers use safety and quality data to inform patient referral preferences, benefit designs, and direct contracting considerations.

For 20 years, The Leapfrog Group has been collecting data on the safety and quality of inpatient hospital care through the annual Leapfrog Hospital Survey. In 2019, they expanded into outpatient care with the launch of the Leapfrog Ambulatory Surgery Center (ASC) Survey. Over 2,100 hospitals and nearly 400 ASCs voluntarily demonstrated their commitment to transparency by reporting the data that purchasers and consumers need to choose the best facility for their care.
