Health Action Council facilitates and participates in initiatives that encourage the use of data-driven and value-based approaches when purchasing healthcare benefits. We also promote the employer perspective by helping providers, insurers, and policy makers to better align payment models and health outcomes.

Here are two examples:

Leapfrog Hospital Safety Scores: Health Action Council has been a partner and supporter of the Leapfrog Group’s healthcare quality initiative since 2005. The Leapfrog Group is widely recognized for its focus on preventable medical errors, patient safety and the health information structure through its annual surveys. As a Leapfrog Rollout Region Leader, Health Action Council encourages Ohio, Minnesota and Oregon hospitals on behalf of our employer members to take part in Leapfrog’s self-reporting survey. We then are able to analyze and provide to our group purchasing members their employees’ utilization of both graded and non-reporting hospitals. Our goal in providing this information is to initiate meaningful conversations with hospitals and employers around the value and quality of care.

Lake Health Primary Care Initiative: Health Action Council members Lubrizol Corporation, Lake County Schools Council and Progressive Corporation are collaborating with Lake Health to provide coordinated, patient-centered care for their employees who receive care from a physician in the Lake Health Integrated Physician Hospital Enterprise (IPHE). Facilitated by Health Action Council, the employer-sponsored primary care initiative supports a delivery model known as a Patient-Centered Medical Home (PCMH) to provide patients with coordinated care and support, especially with regard to chronic conditions.

Health Action Council continues to seek opportunities to convene employer, provider and community discussions that are focused on community health, and how these stakeholders can begin to bridge gaps to co-create something new.
