Chronic conditions account for 90% of all healthcare expenditures and these costs continue to climb. 

Using a mix of machine learning, thousands of clinical rules, and omni-channel outreach, we can identify members early in their care journey to proactively engage them through their preferred channels, reducing medical costs and improving health outcomes for all. Members receive actionable interventions that help them avoid unnecessary Emergency Department visits by helping them close gaps in care and providing site of care recommendations.
