Telemedicine promises immediate access to healthcare whenever and wherever patients need it, while lowering costs by avoiding expensive in-person visits and overhead. To date, telemedicine solutions have focused on a small subset of healthcare spend drivers (urgent care, critical care, and mental health) while leaving one of the largest cost drivers mostly unaddressed: chronic disease.

Chronic conditions are difficult to address with telemedicine because they are continuous, not episodic; require specialized medical attention that includes ongoing adjustments to care plans; and hinge heavily on patients' daily behaviors, such as eating habits and medication adherence. Continuous Remote Care solves these problems with three key components not included in the typical episodic telemedicine model: continuous biomarker monitoring, a dedicated consistent care team, and behavior activation engine.
