90% of total health care costs are related to members with chronic conditions and 60% of adults have more than one chronic condition. Chronic conditions are managed with medications, yet 50% do not take medications as prescribed. And, 40% of treatment failures are driven by medications that are not optimized. 16% of total US health care spend is related to non-optimized medication use.

Uncovering the “why” behind non-optimized drug use improves health and reduces overall health care costs. Chronic Condition Management (CCM) provides a personalized, comprehensive care plan for patients, which leads to better management of disease states. Without an effective CCM strategy, employers’ high-risk population will continue to face challenges in managing their health with personal and financial obstacles which increases overall costs for plan sponsors.

This presentation will focus on the cost improvements of effective chronic condition management and the impact it has on total health care costs over time. This includes an effective strategy for identification, engagement and measurement as well as the importance of treating the whole patient, rather than individual disease states.
